Most contractors sub the millwork out. We don't. We run a shop in Los Angeles staffed by carpenters we've grown in-house, drawing to the architect's spec and fabricating against shop drawings produced by our own draftsmen. The team that draws it builds it - and the team that builds it installs it. Design intent doesn't get translated through three vendors before it lands on site.
